W3RT Neighbours Together project work very closely with the Link Workers, Community Navigators and Social Prescribers who provide Social Prescribing support in the #Watford and #Threerivers area.
 
So what is "Social prescribing"? It is where health professionals refer patients to support in the community, in order to improve their health and wellbeing. So support around Loneliness and Isolation is key aspect of their role.
